New Patient Registration
Anyone living within the Practice area is welcome to register with us and may do so by contacting a member of the administration team. If you change address please check with reception if your new address is still within our practice boundaries.
As a patient, we will endeavour to provide you with the best possible health care. In return we would ask that you be courteous to our staff at all times and that you give consideration to other patients who may have more pressing problems. Please do not make unnecessary requests for home visits as they are extremely time consuming and please let us know if you are unable to attend an appointment so it may be reallocated. We reserve the right to remove you from our list should your behaviour be abusive or violent.
We do not discriminate on the grounds of a patient’s race, creed, sexual orientation or life-style.

Registering With Us
In order to register with us, please collect the relevant forms from the surgery. When you register you will be asked to fill out a medical questionnaire. This is because it can take a considerable amount of time for us to receive your medical records.
Alternatively, you can download these forms using the links below. Please print and fill in BOTH forms with as much detail as possible. Once complete, please bring the forms to the surgery, along with photographic ID. Please note: for those aged under 16, a medical questionnaire is not required, however the GMS1 form is still a requirement.
Completion of these forms does NOT automatically register you with the practice and acceptance of your registration is NOT guaranteed.
We do not offer an online ‘pre-registration’, as we still require you to attend with ID and fill out our registration forms, regardless of whether you pre-register online. This allows us to cut down the time it takes for you to register with us.
