Job description

Court Street Medical Practice is a welcoming, patient orientated practice which believes maintaining the patient-doctor relationship is fundamental to general practice. We pride ourselves on knowing our patients enabling us to put their healthcare into context.

We are looking to recruit a full-time ANP or ACP. Someone looking to join an expanding, forward thinking practice recently located to new bespoke building located near the Ironbridge Gorge and Shropshire hills.

Our clinicians are extremely knowledgeable and supportive of each other. We work closely as a team providing high quality, personalised care to over 7000 patients and benefit from very low staff turnover with enlightened partner attitudes to healthcare, patient care, staff care and community involvement.

Our Social Prescribing model has led the way in giving patients the support they need rather than simply medical intervention. In doing so, we have managed to maintain demand at a sustainable level even as our patient list has grown.

Based in the practice is Telford Mind, the local affiliate of the mental health charity Mind. This gives us seamless access to mental health support for patients (and staff). We work extremely closely with Telford Mind, involving them where possible in decision making and the care needs of our patients.

Currently a two-partner practice with three salaried doctors, the practice is looking to expand its clinical capacity. We have a nursing staff of two full-time nurses and two full-time HCAs.

In addition, we are a member of progressive Primary Care Network with opportunities for the right person to help shape the future of primary care. We host the PCN Multi-Disciplinary Team and work closely with other PCN members.

We take continuity of care very seriously, have excellent QOF attainment and a CQC rating of ‘Good” across all areas. In addition, we have a dedicated admin staff, host a range of activities and services from Quit Smoking to walking groups and activities for older people.
